Surge in "Big Data" Tools Transforms Population Health Management: Study

The use of EHRs for population health management increased five-fold from 2012 to 2014, according to a new downloadable HINtelligence report.

Sea Girt, NJ, September 04, 2014 --(PR.com)-- As population health management (PHM) finds its footing in value-based healthcare, PHM program focus, support tools and key players continue to shift, according to the 2014 Population Health Management Survey by the Healthcare Intelligence Network.

In the two years since HIN last administered this survey, the use of data analytics tools barely on programs' radars in 2012 has surged: the use of health risk assessments (HRAs), registries and biometric screenings more than tripled in the last 24 months, while electronic health record (EHR) applications increased five-fold for the same period.

PHM team composition has also shifted to reflect a greater reliance on primary care: primary care physicians (PCPs) appeared on 86 percent of respondents' PHM teams, versus 60 percent in 2012. The presence of case managers has grown more than 15 percent; registered dieticians and social workers also have more skin in the PHM game in 2014.

More results from this second comprehensive survey are available in the latest HINtelligence report, Population Health Management in 2014: Surge in 'Big Data' Tools Transforms Care Coordination, a complimentary downloadable HIN white paper.

News Facts: HIN's white paper, Population Health Management in 2014: Surge in 'Big Data' Tools Transforms Care Coordination, summarizes June 2014 responses from 129 hospitals/health systems, health plans, primary care practices and others on their application of population health management.

Other data highlights from the survey include the following:

The prevalence of population health management programs remained relatively stable from 2012 to 2014, with just over half of respondents reporting PHM initiatives.

Nearly two-thirds of 2014 respondents—64 percent—identified the HRA as the primary tool for determining appropriate PHM intervention levels, replacing claims data, the preferred stratification tool in 2012.

Nearly one-third of responding PHM programs belong to a medical neighborhood, a new metric derived from this year's survey.

Quote attributable to Melanie Matthews, HIN Executive VP and COO:
"Industry analysts have identified population health management as an area ripe for growth. As indicated by this year's findings, the explosion of data analytics tools has only fueled the growth of this field for identifying high-risk, high-cost individuals and tracking their progress as they learn to manage their health and chronic conditions in a more value-oriented healthcare system."
