Visitors will find a list of the most used forms including enrollment and claim forms from popular carriers such as Blue Cross Blue Shield of Michigan, Blue Care Network of Michigan, AFLAC, Total Health Care, Mutual of Omaha, Colonial Life, Guardian and Prudential. These forms can easily be accessed by simply visiting the website, selecting the “Commonly Used Forms” tab, and clicking the desired form. The forms are all available in pdf making them easy to read and print. Hoover and Associates is also more than willing to assist customers with filling out forms or answering any questions related to the forms.
