Dr. Thomas Murry

Dr. Thomas Murry, Professor of Speech Pathology in Otolaryngology, Department of Otolaryngology, Weill Cornell Medical College gave his talk on Chronic cough: a laryngeal dysfunction. His talk highlighted the assessment of laryngeal function in patients with unresolved chronic cough despite being previously treated. The onset of chronic cough is often unknown and the number of treatments tried is often directed at problems of the nasopharynx or gastroenterological system. The larynx is often overlooked. Non-pulmonary chronic cough may be diagnosed in patients who complain of shortness of breath, choking, and voice disorders. When the larynx is examined, clinicians may observe a normal larynx, inflammation, and/or the presence of paradoxical vocal fold motion. The complaints by the patients may be attributed to clinical observations or they may range from dysphagia, post nasal drip or gastroesophageal reflux disease. Factors such as duration of the condition, timeline of which symptom actually came first, and cooccurrence of other conditions make the diagnosis and the treatment a challenge. These factors may include neurological changes, post nasal drip, upper respiratory infections or pulmonary disease. In most cases, the true cause of chronic cough is never identified. Moreover, without a definitive diagnosis, treatment often fails and the patient is left to seek other options or even just simply they live with it. In many cases, patients report partial resolution of the problem and stop seeking treatment. In others, medications help to diminish the problem and the patient is satisfied with partial resolution. All too often, these medications offer short term help but after a period of time the symptoms return.

Richard J. Wiet

Richard J. Wiem, Professor of Clinical Otolaryngology and Neurosurgery at Northwestern University and Senior Partner, Ear Institute of Chicago, LLC gave his talk on History and newer management strategies in acoustic neuroma management. Here is a bird’s view on his talk. Vesitbular schwannomas are the most common CPA tumor representing 6%-10% of intracranial neoplasm. Incidence is 10 per million per year in the US. NF2 represents about 5% of these CPA tumors. Among these cases 10% will present with normal hearing, and 90% will present with unilateral hearing loss and tinnitus. ABR is a noninvasive test for acoustic neuroma with a fairly high false potential rate. However, MRI is still the gold standard in diagnosis of tumors 2mm in size or larger. Three management strategies: observation, radiation, and surgery are the options for patients and each were discussed on the basis of management of more than 1500 cases seen over a thirty year period. He also focused his talk on recent conservative trends in management of the acoustic neuroma patient in the past decade.
