Heart House New Jersey Launches Virtual Personal Concierge for Heart Failure Patients Using GoMo Health’s BehavioralRx®
Asbury Park, NJ, November 09, 2018 --(PR.com)-- The Heart House, a New Jersey based cardiology practice, and GoMo Health today launched their digital Personal Concierge for patients with systolic and diastolic heart failure, including co-morbidities of diabetes, renal failure and smoking. The program integrates American Heart Association digital content.
The Personal Concierge ePRO system (electronic patient reported outcomes) uses GoMo Health’s BehavorialRx® science to engage the patient and caregiver, helping them manage medications, cope with anxiety and stress, and manage their movement and healthy diet. It provides real-time triage and escalation to nurses based on patient responses or indications of weight gain, shortness of breath, edema and medication refill status.
