The Sidney Kimmel Cancer Center at Jefferson will use a novel platform to delivery survivorship care plans. By using Carevive’s Care Planning System (CPS), Jefferson will be able to provide personalized survivorship care plans to its patients across multiple tumor types. The Carevive CPS will integrate with Jefferson’s electronic health record (EHR) and registry software.

The Sidney Kimmel Cancer Center at Jefferson will use a novel platform to delivery survivorship care plans. By using Carevive’s Care Planning System (CPS), Jefferson will be able to provide personalized survivorship care plans to its patients across multiple tumor types. The Carevive CPS will integrate with Jefferson’s electronic health record (EHR) and registry software.
Registry data can be used to automate the Treatment Summary. Patient and PCP versions of care plans can be generated and Carevive’s reporting functionality are used to improve care coordination across disciplines.
