Carevive to Kickoff Survivorship Care Planning at Jefferson University

The Sidney Kimmel Cancer Center at Jefferson will use a novel platform to delivery survivorship care plans. By using Carevive’s Care Planning System (CPS), Jefferson will be able to provide personalized survivorship care plans to its patients across multiple tumor types. The Carevive CPS will integrate with Jefferson’s electronic health record (EHR) and registry software. - March 21, 2016
